Participant Consent Form (template)
Template for tournament organizers using fencing.ru. The organizer is the data controller; fencing.ru processes data on the organizer's behalf. The “Organizer and competition” block is filled in once by the organizer (it can be pre-printed on all forms). The participant fills in only their name, signature, and date. Collect this from each participant (or their parent / legal guardian for minors) before the competition is published. Written or electronic records (online form, simple e-signature) are both acceptable if verifiable.

full name; year of birth; region; sports club / organization; sports rank / title; age category; coach's name; competition results (bout scores, placements, participation status).
- Purpose: organizing and running the competition above — start lists, draws, protocols, final results.
- Processing system: the fencing.ru competition service (Sole Proprietor Sergei Tikhomirov, Russian Federation), acting as the organizer's processor. Data is processed on servers located in the Russian Federation.
- Duration / withdrawal: until the purpose is fulfilled or until withdrawn. I may withdraw at any time; withdrawal does not affect prior processing. I am aware of my rights of access, rectification, erasure, restriction, and complaint to a supervisory authority.
Publication. I additionally consent ☐ / do not consent ☐ to the public online publication of the above data (live protocols, start lists, brackets, final results, venue scoreboards) on fencing.ru, on the partner platform (zenfencing.ru), and on the organizer's channels named above, accessible to an unlimited audience. (The participant's name and scores are essential to a public competition protocol; without this consent the organizer must not publish the optional details — region, club, rank, year of birth, coach.)
